Accident Investigation

Using the WorkSafe BC website, you will examine several safety scenarios, including some accidents in which workers have been hurt or fatally injured.

You can find the website here. It is essential for all of these sections.

Part I - Overview / Main Points

Use MS Word to do the following tasks.

You will select five different Audio Slide Shows and do all of the following for each... (in other words, you will do this five separate times... once for each of five safety topics).

  1. Record the title
  2. Record the presenter
  3. Write down what you believe is the main idea.
  4. Write down three other key facts/ideas.
  5. Include one take-away question you have after watching the video.
    • It could be a question you would still like answered or,
    • Something you would ask if working in a similar situation in the future.

Your write up should be based on the key safety components. Try to apply what you have learned from your explorations of workplace safety and from good safety practices in general.

Part II - Safety Quiz

Select one Audio Slide Show (same link as above) or one of the videos from the featured video page and create a quiz in PowerPoint that your classmates could answer.

It should include the following:

  1. Title slide
  2. Link to the Audio Slideshow or video on which it is based (so the person taking your quiz can watch it)
  3. Ten questions, each presented on a different slide
  4. The answers, which are revealed after the test taker selects an option.
  5. A final slide (a "The End" type slide)

Your questions should be based on safety concerns. Avoid questions which are obvious or questions which are not relevant to the safety message being conveyed.

You can select one of the Audio Slide Shows you covered in the previous activity. Be sure to select one that will be able to provide you with an appropriate number of questions.
